Coverage

Due to the changing insurance environment, and amount of control of treatment dictated by insurance companies when in contract with a physical therapist, it is OHM PT’s belief that we can provide superior care by remaining out of network. This allows OHM to dictate therapy plans of care based on what is best for our patients, not what is dictated by reimbursement. You will be provided a superbill which can be submitted to your insurance for reimbursement. HSA/FSA payments accepted.

Evaluation

75 minutes/$150 - one-on-one evaluations with a doctor of physical therapy will allow for a greater understanding of what’s going on with your body. Gain the knowledge you need to becoming an active problem solver and take control of what’s causing you pain. Manual therapy and corrective exercise included in appointment.

Treatment Sessions

60 minutes/$130 - Treatment sessions include manual therapy, mobilizations, and corrective exercise to address impairments found during evaluation. With a focus on functional mobility and activity-specific training, we are able to help you progress toward your goals. Packages of sessions can be purchased at a discounted rate:

4 follow-up sessions: $480 ($120/visit)

8 follow-up sessions: $920 ($115/visit)

12 follow-up sessions: $1320 ($110/visit)

Virtual Appointments

45 minutes/$80 - Now offering evaluation and treatments from the comfort of your own home. Evaluation, plan of care development and treatment can all be performed remotely. Learn how to perform various techniques and corrective exercises to address your specific issue.
